Spondylolysis is an overuse injury affecting the pars interarticularis—the thin bridge of bone connecting the joints at the back of a vertebra.  It starts as an early stress reaction (inflammation and micro-cracks from repetitive bending or twisting) and can progress to a full stress fracture if unhealed.

L5 is the fifth and lowest lumbar vertebra, located right above the sacrum near the beltline. In fact, over 85–90% of all spondylolysis cases occur at this specific vertebra because it carries the most weight and stress.

The gold-standard treatment is non-surgical conservative care:

Rest:
(1) Pause sports/back-bending for 6–12 weeks so the bone heals,
(2) Bracing: Wear a lumbar brace if prescribed to immobilize the spine,
(3) Physical Therapy: Target core and hamstring strength to stabilize the lower back.
