Synovial Cyst, Lumbar Spine

A synovial cyst at the L4–L5 lumbar spine is a small, fluid-filled sac that forms in the facet joint of the lower back, usually due to arthritis or age-related wear. The cyst develops when the joint produces excess lubricating fluid, and it can press on nearby spinal nerves. Some people have no symptoms, but others may experience lower back pain, sciatica (shooting pain down the leg), numbness, tingling, or weakness, especially when standing or walking. Diagnosis is usually made with an MRI. Treatment depends on severity: mild cases may be managed with rest, physical therapy, medications, or injections, while more serious cases that cause significant nerve compression may require surgery to remove the cyst, and sometimes stabilize the spine.
