There is no neoprene in the mat or the belt. It is composed of 3M conductive rubber
Marks are the contact point to the persons back, and the marks indicate small rash/burn spots caused by the mats electrolysis, which can affect some people, but not everyone.
If the customer was wearing a T-shirt, then perhaps it is too thin? However the description provided appears to indicate the customer is using the mat directly on the skin.
The spot may even develop the same shape as the mat geometry which might make a person think its a material transfer mark, where it is in fact a electrolysis effect of the mat on sensitive skin
Avoid using the mats/belts directly on skin if observing a mark of any kind on the skin. Always wear a light T-shirt, or simply place the Mat under the beds cotton bottom sheet, and this would suffice.
